Post-Step Therapy Prior Authorization Criteria

Covered with post-step therapy prior authorization when ANY of the following are met:

Coverage conditions: The patient has experienced an inadequate treatment response after at least a 30-day trial of at least one brand or generic over-the-counter (OTC) nasal steroid or at least one generic prescription nasal steroid or at least one generic prescription nasal steroid combination.30 days

Supportive documentation required

Coverage conditions: The patient has a documented contraindication or a potential drug interaction that would prohibit a trial of at least one brand or generic OTC nasal steroid or at least one generic prescription nasal steroid or at least one generic prescription nasal steroid combination.

Document contraindication or interaction

Coverage conditions: The patient has experienced an intolerance to at least one brand or generic OTC nasal steroid or at least one generic prescription nasal steroid or at least one generic prescription nasal steroid combination.

Claim rejection when step therapy not met

If the patient does not meet the initial step therapy criteria, the claim will reject with a message indicating that a prior authorization (PA) is required. Providers must submit a PA request for the requested branded nasal steroid or branded nasal steroid combination; the PA unit will then apply the prior authorization criteria to the request.

Initial step therapy requires a trial (or documented contraindication/intolerance) of at least one OTC brand or generic nasal steroid, or at least one generic prescription nasal steroid or generic prescription nasal steroid combination prior to coverage of a branded nasal steroid or branded nasal steroid combination without PA. The trial should be at least 30 days unless a contraindication or intolerance is documented.
