Policy summary
Neighborhood Health Plan expects providers and dispensing pharmacies serving members in assisted living and long-term care facilities to use Neighborhood drug formularies when creating drug treatment plans. The plan requires that if a member needs a drug not on their respective formulary — including non-formulary over-the-counter (OTC) drugs or medications that have a utilization management (UM) restriction — the provider must submit a Prior Authorization to Neighborhood for approval before the medication is dispensed.
Dispensing pharmacies serving assisted living and long-term care members may request that providers submit a Prior Authorization for drugs with UM restrictions (including non-formulary and OTC drugs) prior to placing an override and dispensing the medication.
