Authorization of 12 weeks may be granted for continued treatment of anemia in rheumatoid arthritis when ALL of the following are met:

Anemia Due to Hepatitis C Treatment

Authorization of 12 weeks may be granted for continued treatment of anemia due to Hepatitis C treatment when ALL of the following are met:

Hepatitis C treatment–related anemia: member is receiving ribavirin in combination with either interferon alfa or peginterferon alfa

AND current hemoglobin < 12 g/dL

Authorization of 12 weeks may be granted for continued treatment of anemia due to zidovudine in HIV-infected patients when ALL of the following are met:

Zidovudine-related anemia: current hemoglobin < 12 g/dL< 12 g/dL

Anemia in Members Whose Religious Beliefs Forbid Blood Transfusions

Authorization of 12 weeks may be granted for continued treatment of anemia in members whose religious beliefs forbid blood transfusions when ALL of the following are met:

Religious refusal of transfusion: member's religious beliefs forbid blood transfusions

AND current hemoglobin < 12 g/dL

Religious refusal of transfusion: current hemoglobin < 12 g/dL< 12 g/dL

Anemia in Primary Myelofibrosis or Related Conditions

Authorization of 12 weeks may be granted for continued treatment of anemia in primary myelofibrosis, post-polycythemia vera MF, or post-essential thrombocythemia MF when ALL of the following are met:

Myelofibrosis-related anemia: diagnosis of primary myelofibrosis, post-polycythemia vera myelofibrosis, or post-essential thrombocythemia myelofibrosis

AND current hemoglobin < 12 g/dL

Myelofibrosis-related anemia: current hemoglobin < 12 g/dL< 12 g/dL
