Coverage Criteria for Mircera (methoxy polyethylene glycol-epoetin beta)

Initial Therapy

Covered when ALL of the following are met:

Initial CKD anemia therapy: 1) Diagnosis: anemia due to chronic kidney disease (CKD) in an adult (on dialysis or not) or a pediatric patient 5–17 years on hemodialysis converting from another ESA after hemoglobin stabilization. 2) Pretreatment hemoglobin < 10 g/dL (values exclude recent transfusion). 3) Adequate iron stores: TSAT ≥ 20% within the prior 3 months or receiving iron therapy. 4) Member is not receiving concomitant erythropoiesis-stimulating agents.

Authorization of 12 weeks may be granted.

Continuation Therapy

Covered when ONE of the following sets is met:

Continuation - established responder: 1) Current hemoglobin < 12 g/dL (values exclude recent transfusion). 2) Member has shown response to therapy with a rise in hemoglobin of > 1 g/dL after at least 12 weeks of ESA therapy. 3) Adequate iron stores: TSAT ≥ 20% within the prior 3 months or receiving iron therapy. 4) Member is not receiving concomitant erythropoiesis-stimulating agents.

Authorization of 12 weeks may be granted.

Continuation - incomplete initial therapy: Members who have not completed 12 weeks of ESA therapy may be granted authorization of up to 12 weeks.

Not Medically Necessary / Experimental

Not covered indications: All indications other than the FDA‑approved indications and compendial uses listed (treatment of anemia due to CKD as specified) are considered experimental/investigational and not medically necessary.

Indications for Mircera that are not included in the FDA‑approved labeling or recognized compendia are considered experimental/investigational and therefore not medically necessary for coverage.

Use of Mircera for any indication not listed in the FDA‑approved indications (treatment of anemia associated with chronic kidney disease in adults, and in pediatric patients 5–17 years on hemodialysis converting from another ESA) will be treated as not medically necessary.

Prior Authorization

Prior Authorization Required

Prior authorization required for initial and continued Mircera therapy. Providers must obtain authorization before starting or continuing Mircera.

Authorization durations: initial and continuation approvals may be granted for up to 12 weeks.
Members may not receive Mircera concurrently with other erythropoiesis-stimulating agents (no concomitant ESA therapy).

Documentation Required

Concurrent ESA Use and Conversion Rule

No concomitant ESA therapy. Mircera must not be used at the same time as other erythropoiesis-stimulating agents. Pediatric conversions: Mircera is indicated for pediatric patients 5–17 years on hemodialysis who are converting from another ESA after hemoglobin stabilization with an ESA.

Pediatric conversion limited to ages 5–17 years on hemodialysis with prior hemoglobin stabilization on an ESA.

Required Laboratory Documentation

Required laboratory documentation: Members must have adequate iron stores prior to initial or continued therapy. Document a serum transferrin saturation (TSAT) ≥ 20% within the prior 3 months or documentation that the member is receiving iron therapy.

Response Documentation for Continuation

Response documentation for continuation. For continuation of Mircera, providers must document a clinical response: a rise in hemoglobin of greater than 1 g/dL after at least 12 weeks of ESA therapy. If the member has not completed 12 weeks of ESA therapy, authorization for up to 12 weeks may be granted while awaiting assessment.

Document current hemoglobin < 12 g/dL (excluding values due to recent transfusion) and a hemoglobin increase > 1 g/dL after ≥ 12 weeks of ESA therapy to qualify for continuation.
If < 12 weeks of ESA therapy completed, authorize up to 12 weeks pending completion and response assessment.
