Added multiple new COVID-19 laboratory CPT/HCPCS codes over time (examples: 0224U, 0240U, 86413, 87428).
Cost-share waiver policy was clarified with different effective rules before and after July 21, 2020.
Modifier 'CS' (Cost-sharing Waived) is accepted on listed laboratory codes.
