This policy documents coverage and documentation requirements for both qualitative (presumptive) and quantitative (definitive/confirmatory) drug toxicology testing. Qualitative testing (immunoassay/screening) and quantitative testing (definitive/confirmatory) are each covered when clinical criteria are met. Urine is the preferred specimen for broad screening because blood is relatively insensitive for many common drugs and saliva has a shorter detection window. Quantitative tests must be laboratory-performed and drug-specific (ordered naming the specific drug(s) to be confirmed). Specimen validity testing (e.g., urinary pH, specific gravity, creatinine, nitrates, oxidants) is considered an internal control and is not separately billable. The policy applies to Medicaid (excluding EFP), Commercial, Dual CONNECT (Coordination only D-SNP), and INTEGRITY for Duals (Fully Integrated D-SNP) lines of business.

Qualitative (Presumptive) Drug Tests - Covered Indications

Qualitative Drug Tests (Presumptive) covered when ANY of the following are met:

ANY of the following

A member is receiving treatment for chronic pain with prescription opioid or other medication associated with increased risk for misuse or addiction
A member is undergoing treatment for substance use disorder or is otherwise requiring monitoring for use/misuse of controlled or illicit substance(s)
Misuse of prescribed or illicit substances is suspected
A member is beginning a pain management program or substance use disorder treatment program

Quantitative (Definitive/Confirmatory) Drug Tests - Covered Indications

Quantitative Drug Tests (Definitive) covered when ALL of the following ordering and indication rules are met (and for but not limited to the listed clinical reasons):

Clinical triggers (any one)
- Unexpected positive test inadequately explained by the patient
- Unexpected negative test (suspected medication diversion)
- Need for quantitative levels to compare with established benchmarks for clinical decision making
Must be ordered on an individual basis by a medical provider directly caring for the member at the time of order (not from standing/routine orders)
Must be ordered with an indication of the specific drug(s) being confirmed (not as a comprehensive confirmatory panel)

General Ordering and Frequency Limitations

Drug screening for medico-legal purposes (e.g., court-ordered drug screening)
Drug screening for employment purposes (pre-employment or continuation of employment)
Qualitative testing required for or part of participation in a substance use disorder treatment program with an all-inclusive bundled rate
Routine testing (i.e., testing at every visit) for qualitative testing
Testing ordered by or for third parties for the sole purpose of meeting the requirements of a third party (qualitative or quantitative)

Quantitative (definitive/confirmatory) tests must be ordered on an individual basis by the provider directly caring for the member and must specify the specific drug(s) to be confirmed (not as a comprehensive panel). Quantitative tests must be performed in a laboratory. Specimen validity testing performed to confirm urine integrity (e.g., pH, specific gravity, creatinine, nitrates, oxidants) is considered an internal control process and should not be separately billed (see Specimen Validity Testing Billing criteria).

Quantitative (definitive) tests require clinical justification. Document unexpected positive or unexpected negative results or the clinical need for quantitative levels to support definitive testing. Tests must be ordered for specific drug(s) to be confirmed.
