This guideline defines accepted indications, continuation rules, exclusions, and coding for Vectibix (panitumumab) for the treatment of colorectal cancer and related off-label contexts for Neighborhood Health Plan of Rhode Island lines of business. Evolent is responsible for processing all medication requests from network ordering providers and enforces prior authorization; medications not authorized by Evolent may be deemed not approvable and not reimbursable.

Accepted Indications and Use Criteria (High-level)

Colorectal Cancer - Indications for Vectibix (panitumumab)

Vectibix is acceptable for members with specified disease and genetic status in any of the following settings:

ALL of the following

AND one of
- Member has KRAS/NRAS/BRAF wild-type disease and Vectibix will be used in combination with FOLFOX (fluorouracil, leucovorin, and oxaliplatin)
- Member has KRAS/NRAS/BRAF wild-type disease and Vectibix will be used in combination with FOLFIRI (fluorouracil, leucovorin, and irinotecan)
- Member has KRAS/NRAS/BRAF wild-type disease and Vectibix will be used as a single agent
- Member has KRAS/NRAS/BRAF wild-type disease and Vectibix will be used in combination with irinotecan for subsequent therapy following prior chemotherapy for metastatic disease
Capecitabine (Xeloda) may be substituted for 5-FU in the above 5-FU-based regimens (operational substitution allowance)

Continuation Requests Exemption Criteria

Continuation requests for a not-approvable medication shall be exempt provided ALL of the following are met:

The requested medication was used within the last year without a lapse of more than 30 days of having an active authorization30 days

Exclusion Criteria (Not acceptable)

Vectibix is not acceptable when ANY of the following exclusion criteria are present:

Absence of documented KRAS/NRAS/BRAF testing and results of such testing for the above indications
Dosing exceeds single dose limit of Vectibix (panitumumab)single dose limit = 6 mg/kg
Investigational use of Vectibix with an off-label indication that is not sufficient in evidence or is not generally accepted by the medical community

For off-label use to be considered sufficient it must meet CMS/compendia/peer-reviewed standards; assess ALL of the following evidence dimensions:
