Initial and Continuation Use — NSCLC

Zykadia (ceritinib) may be used as monotherapy in adult members for first line or subsequent therapy of ALK rearrangement positive metastatic NSCLC when ALL of the following are met:

Continuation Therapy Criteria — Exemption

Continuation requests for a not-approvable medication shall be exempt from this Evolent policy provided ALL of the following are met:

No progression: Member has not experienced disease progression on the requested medication
Recent use without lapse: Requested medication was used within the last year without a lapse of more than 30 days of having an active authorization
No additions: No additional medication(s) are being added to the continuation request
