- Prior therapy includes an anti-CD38 monoclonal antibody (e.g., daratumumab, isatuximab)

Continuation Authorization Criteria

Continuation requests for a not-approvable medication shall be exempt from this Evolent policy provided ALL of the following are met:

Ensure Elrexfio is administered only under the ELREXFIO REMS program. Initiate treatment with the recommended step-up dosing schedule to reduce CRS risk. Monitor patients for cytokine release syndrome (CRS) and neurologic toxicity including ICANS; withhold the drug until toxicity resolves or permanently discontinue based on severity.
