Use of these agents must be supported by FDA-approved product labeling, CMS-recognized compendia, NCCN or ASCO clinical guidelines, or acceptable peer-reviewed literature that meets CMS Medicare Benefit Policy Manual Chapter 15 criteria.

High-level clinical warnings and rationale from product labeling include boxed warnings that ESAs increase the risk of death, myocardial infarction, stroke, venous thromboembolism, and thrombosis of vascular access. In CKD, trials showed increased risks when targeting hemoglobin > 11 g/dL. In cancer, ESAs have been associated with shortened overall survival and/or increased tumor progression in several cancer types; therefore ESAs should be used only for anemia from myelosuppressive chemotherapy (not when cure is intended) and at the lowest dose sufficient to avoid RBC transfusion.
