Initial authorization criteria

Lotronex initial coverage criteria: 1) Patient is a biological female or self-identifies as female; 2) Diagnosis of severe diarrhea-predominant irritable bowel syndrome (IBS); 3) Chronic IBS symptoms lasting at least 6 months; 4) Anatomic or biochemical abnormalities of the gastrointestinal tract have been excluded; 5) Inadequate response to conventional therapy.>= 6 months

Severity defined as diarrhea plus one or more: frequent and severe abdominal pain/discomfort; frequent bowel urgency or fecal incontinence; or disability/restriction of daily activities due to IBS.

Use of alosetron (Lotronex) in men is not supported by adequate clinical studies and is outside the FDA‑approved indication. The FDA‑approved labeling specifies that clinical studies have not been performed to adequately confirm benefit in men, and the indication is limited to women (biological females or those who self‑identify as female) with severe diarrhea‑predominant IBS. Requests for Lotronex for male patients are therefore not consistent with the FDA‑approved indication.
