Medical Necessity and Coverage Criteria

Initial coverage criteria

Covered when ALL of the following are met

Age: Member is 11-18 years of ageage = 11-18 years

Diagnosis of a ROME IV defined functional gastrointestinal disorder (functional abdominal pain, functional abdominal syndrome, irritable bowel syndrome, functional dyspepsia, or abdominal migraine) with painROME IV criteria met

Symptom duration: Symptoms present for at least 9 monthsduration >= 9 months

Organic disease exclusion: Organic disease has been excluded and documentation submitteddocumented exclusion of organic disease

Attach documentation demonstrating exclusion of organic disease

Diet trial: Trial and failure of diet modificationdocumented diet modification trial and failure

Document type and duration of dietary interventions tried

Medication trials: Trial and failure of at least one medication in each of the following three categories: (1) acid suppression (H2 blocker or PPI); (2) neuromodulators (e.g., amitriptyline, nortriptyline, gabapentin, periactin, aprepitant); (3) antispasmodics or motility agents (e.g., hyoscyamine, dicyclomine, erythromycin, linaclotide, prucalopride)failed medications in all 3 categories

Document medication names, doses, durations, and reason for discontinuation or lack of efficacy

The following conditions are absolute exclusions for IB‑Stim therapy: a history of cardiac pacemaker, hemophilia, or psoriasis vulgaris. Requests for IB‑Stim in members with any of these histories will not be covered.
