Coverage Criteria for Durable Medical Equipment (DME)

General coverage criteria for DME

Covered when ALL of the following are met

Basic DME coverage criteria: Item is primarily and customarily used to serve a medical purpose, is appropriate for use in the patient’s home, is not generally useful in the absence of illness or injury, and is durable (can withstand repeated use).

Definition derived from policy; aligns with CMS/DHS guidance

Ordering and episode-of-care: DME is ordered by a practitioner or delivered/utilized by a practitioner during an episode of care; both primary care and specialty practitioners may order or utilize DME; acceptable places of service include office (POS 11), home (POS 12), urgent care (POS 20), inpatient (POS 21), outpatient (POS 22), and CHC (POS 50).

Quantity, coverage and authorization: Coverage and quantity limits are established per individual HCPCS code based largely on industry standards with modifications as needed and using CMS/DHS guidance where applicable; items listed on the plan’s non‑covered list are excluded; certain conditionally covered items require prior authorization per the Clinical Medical Policy.

Items explicitly listed on Neighborhood Health Plan of Rhode Island's non-covered DME list (Table 1) are excluded from coverage. The non-covered list is approved by the Chief Medical Officer or designee and is based on CMS Medicare guidelines, DHS guidance, and Neighborhood review. Examples of HCPCS codes referenced in the plan's non-covered or conditionally covered lists include A9270 (Non-covered item or service), A9300 (Exercise equipment), and a range of other A-, C-, E-, L-, and T-codes enumerated in the HCPCS list (see coding section for the full code set).

DME that is not primarily used for a medical purpose, is not appropriate for home use, or would be useful in the absence of illness or injury is excluded from coverage. Neighborhood defines DME as equipment that is primarily and customarily used to serve a medical purpose, appropriate for home use, generally not useful without illness or injury, and capable of repeated use; items failing these criteria do not meet DME medical necessity and are therefore non-covered.
