Anesthesia codes include the full anesthesia care package; services billed with an anesthesia code are bundled unless criteria for independence are met:

Primary anesthesia CPT/HCPCS codes encompass preoperative evaluation, standard preparation and monitoring, administration of anesthesia, intraoperative care, and post-anesthesia recovery care until the patient is transferred by the anesthesia practitioner.
Unless specifically indicated that charges are separate and distinct from the anesthesia provided during the surgical procedure, services billed for preparation, monitoring, and procedural services are assumed included and will be bundled into the primary anesthesia code.

Criteria for separate reimbursement

A service billed with an anesthesia code may be reimbursed separately only when ALL of the following are met:
