Office/point-of-care and microscopy: Testing of pH, testing for the presence of amines, measurement of sialidase activity, saline wet mount, potassium hydroxide (KOH) wet mount, and microscopic examination of vaginal fluids
Point-of-care and office microscopy methods are included
Direct probe DNA-based identification of Gardnerella, Trichomonas, and Candida (e.g., BD Affirm™ VPIII)
Vaginal culture for Candida: Vaginal cultures for Candida species when wet-mount preparations are negative and vaginal pH test is normal, for diagnosis of vulvovaginal candidiasis
NAAT/PCR for Trichomonas: Nucleic acid amplification testing (NAAT) or PCR-based identification of Trichomonas vaginalis

Background and Evidence

Vaginitis is a clinical syndrome most commonly caused by BV, VVC, and Trichomonas vaginalis and is diagnosed using a combination of symptoms, physical exam, and laboratory or point-of-care tests. Initial evaluation typically includes measurement of vaginal pH, amine ('whiff') testing, and microscopic examination (saline and KOH wet mounts) to identify clue cells, motile trichomonads, or yeast forms. Traditional microscopy and Amsel/Nugent criteria remain important rapid diagnostic approaches but have variable sensitivity. Molecular assays (NAAT/PCR/TMA) generally demonstrate higher sensitivity and specificity than microscopy or culture for many targets, provide improved detection of coinfections, and have become accepted diagnostic options for symptomatic women — for example, NAATs are the preferred standard for Trichomonas and multiple validated NAAT-based panels (Aptima BV/CV/TV, BD MAX Vaginal Panel, and others) show higher diagnostic performance compared with clinician assessment or point-of-care tests. However, molecular methods may detect organism colonization in asymptomatic individuals (notably PCR detection of Candida), so their use is recommended primarily in symptomatic patients.
