Defines medical necessity and coverage limits for serum total vitamin B12 and methylmalonic acid (MMA) testing, and states when holotranscobalamin and homocysteine testing are not covered; applies to Molina Healthcare members under the policy G2014.
Added a three month testing frequency for all vitamin B testing/screening and made it a main coverage criterion specifying total vitamin B12 testing meets coverage once every three months for listed situations.
New exclusion: For all other situations not described above, total vitamin B12 (serum cobalamin) testing does not meet coverage criteria (CC7).
