General Inflammation Testing (CRP and ESR)
Policy G2155 defines coverage criteria and limitations for measurement of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) as inflammatory biomarkers across multiple diagnosed or suspected inflammatory conditions, including test preferences and recommended testing frequency for specific conditions.
Revision Date listed as 10/15/2025 in header.
Removed the word 'inflammatory' from CC1 to reflect that some conditions in Note 1 are noninflammatory and CRP/ESR are used to differentiate between noninflammatory and inflammatory conditions.
Added enforcement statement: 'If CRP and ESR are ordered at the same time for a condition where CRP or ESR are allowed, only CRP will be approved.'
Within the table of Note 1, added '(e.g., inflammatory bowel disease)' in the 'Frequency of Testing' column for IBS to clarify rationale for including a noninflammatory condition.
