PTP edits apply to professional and outpatient institutional claims on a prepayment basis; when two related procedure codes are billed by the same provider on the same date, only the most comprehensive code is reimbursable.

Modifier Submission and Clinical Validation Requirements

Submit applicable NCCI modifiers with supporting clinical documentation; expect prepayment validation

Submit claims with applicable NCCI modifiers and include supporting clinical documentation showing the clinical circumstance that justifies modifier use. Claims with valid NCCI modifiers will undergo prepayment clinical validation; the clinical review team will determine whether the modifier is clinically appropriate and may only allow an override where the modifier indicator permits. Providers must not use modifiers solely to bypass edits.

Include documentation demonstrating the patient's clinical situation that supports modifier use.
Be aware each PTP edit has a modifier indicator: 0=no modifier allowed; 1=modifier may be used with supporting documentation; 9=edit deleted and modifier not applicable.
Claims with modifiers are subject to prepayment clinical validation and may be denied if the modifier is not clinically appropriate.
