Coverage Determination
Not Medically Necessary / Insufficient Evidence
Not covered due to insufficient evidence
Policy statement found in document.
It is the policy of Health Net of California that there is insufficient evidence to support the efficacy of prolotherapy for musculoskeletal pain or any other indication.
Although prolotherapy has been evaluated in the literature, available randomized placebo-controlled trials are few and have methodological limitations (including problems with blinding) and have not consistently demonstrated improvement in pain or function. Therefore current evidence is insufficient to show meaningful improvement in health outcomes and does not support routine use of prolotherapy.
