Defines medical necessity criteria, limitations, and coding guidance for selective nerve root blocks (SNRB) and transforaminal epidural steroid injections (TFESI) for acute and chronic radicular pain, including procedural limits, steroid particulate requirements for cervical procedures, and diagnostic/therapeutic indications.
