Coverage Criteria for Belimumab (Benlysta)

Initial Therapy — Systemic Lupus Erythematosus

Covered when ALL of the following are met

SLE initial criteria: 1. Diagnosis of systemic lupus erythematosus (SLE); 2. Prescribed by or in consultation with a rheumatologist; 3. Age ≥ 5 years; 4. Documentation confirming member is positive for an SLE autoantibody (e.g., ANA, anti-dsDNA, anti‑Smith, anti‑RNP, anti‑Ro/SSA, anti‑La/SSB, antiphospholipid antibody); 5. Prescribed in combination with standard therapy for SLE (e.g., glucocorticoids, antimalarial, or non‑biologic immunosuppressant) unless all are contraindicated; 6. Not prescribed concurrently with Lupkynis or another biologic (examples include rituximab, Gazyva, Saphnelo); 7. Request meets one of the following dosing limits: a) IV: does not exceed 10 mg/kg per dose every 2 weeks for the first 3 doses and every 4 weeks thereafter; or b) SC: dosing per weight does not exceed (i) for pediatric members ≥ 40 kg and adults: 200 mg once weekly; (ii) for pediatric members 15 kg to < 40 kg: 200 mg once every 2 weeks.

Initial Therapy — Lupus Nephritis

Covered when ALL of the following are met

LN initial criteria: 1. Diagnosis of lupus nephritis (LN) with kidney biopsy confirming Class III, IV, or V; 2. Prescribed by or in consultation with a nephrologist or rheumatologist; 3. Age ≥ 5 years; 4. Confirmed diagnosis of SLE; 5. Prescribed in combination with standard therapy for SLE/LN (e.g., glucocorticoids, antimalarial, or non‑biologic immunosuppressant) unless all are contraindicated; 6. Not prescribed concurrently with Lupkynis or another biologic (examples include Gazyva, Saphnelo, rituximab); 7. Request meets one of the following dosing options: a) IV: does not exceed 10 mg/kg per dose every 2 weeks for the first 3 doses and every 4 weeks thereafter; or b) SC: (i) for pediatric members ≥ 40 kg and adults: 400 mg once weekly for the first 4 doses (induction), then 200 mg once weekly; (ii) for pediatric members 15 kg to < 40 kg: 200 mg once weekly for the first 4 doses, then 200 mg once every 2 weeks. Note: loading/induction doses are not permitted if previously receiving belimumab for SLE.

Continued Therapy — All Indications

Continued coverage when ALL of the following are met

continued therapy criteria: 1. Member is currently receiving belimumab via the benefit or has previously met initial approval criteria, or continuity of care applies; 2. Clinical response: a) For SLE: member is responding positively to therapy; b) For LN: member is responding positively as evidenced by one of: i. UPCR ≤ 0.5 mg/mg from baseline with low‑dose steroids; ii. No reduction from baseline in eGFR > 20% with low‑dose steroids; iii. eGFR ≥ 60 mL/min/1.73 m2 with low‑dose steroids; 3. Continued concomitant standard therapy for SLE/LN unless contraindicated; 4. Not prescribed concurrently with Lupkynis or another biologic (e.g., Gazyva, Saphnelo, rituximab); 5. If request is for a dose increase, new dose must not exceed specified maximums: a) IV: 10 mg/kg every 4 weeks; b) SC: for pediatric members ≥ 40 kg and adults: 200 mg once weekly; for pediatric members 15 kg to < 40 kg: 200 mg once every 2 weeks.

Dosing and administration — SLE and LN

Non‑FDA approved indications that are not expressly addressed by this policy are excluded from coverage unless the provider submits sufficient documentation of efficacy and safety per the applicable off‑label use policies (see CP.CPA.09 for Commercial, HIM.PA.154 for Marketplace/ICHRA, and CP.PMN.53 for Medicaid) or other evidence of coverage documentation.

Coverage will not be authorized for members with autoantibody negative SLE. Clinical trial data demonstrated no meaningful benefit of belimumab in autoantibody negative patients, and efficacy was confirmed only in autoantibody positive cohorts; therefore requests for autoantibody negative SLE should be denied under this policy.

Concurrent treatment with Lupkynis or another biologic is an exclusion and will not be authorized. The policy explicitly prohibits concomitant use of belimumab with Lupkynis or other biologic therapies (examples now include Gazyva), and prior authorization requests that document concurrent biologic therapy may be denied.

Autoantibody negative SLE and other non‑FDA approved indications without sufficient supporting documentation are excluded per the off‑label use policies referenced in this policy. Providers seeking coverage for such indications must submit documentation that meets the standards in the applicable off‑label policy (commercial, HIM/ICHRA, or Medicaid) to be considered.

Provider must submit documentation (such as office chart notes, laboratory results, or other clinical information) that supports the member meets all applicable approval criteria including diagnosis, prescriber specialty, age, autoantibody status, and concomitant therapy.

Prior authorization request must demonstrate ALL initial approval criteria are met (diagnosis, prescriber specialty, age, autoantibody positive status where required, and concomitant standard SLE/LN therapy).
Provider must include supporting documentation: office chart notes, lab results (e.g., autoantibody tests), biopsy reports for LN when applicable, and any other clinical information relevant to the request.

Background

Belimumab (Benlysta) is a B‑lymphocyte stimulator (BLyS)–specific inhibitor approved for treatment of active systemic lupus erythematosus and active lupus nephritis in patients aged ≥ 5 years, to be used in combination with standard therapy. It is available for intravenous administration (IV) and as a subcutaneous (SC) autoinjector/syringe; IV dosing is weight based and SC dosing varies by age and weight. The drug is not recommended for severe active central nervous system lupus, and clinical trial evidence supporting benefit was strongest in autoantibody positive patients.

Benlysta will not be authorized for certain situations; requests meeting these triggers should be denied per policy.

Autoantibody negative SLE is an explicit noncoverage trigger — coverage is not authorized for autoantibody negative SLE.
Non–FDA approved indications not addressed in this policy are not authorized unless supported by the plan’s off‑label use policies (CP.CPA.09, HIM.PA.154, CP.PMN.53) or evidence of coverage documents.

Denial Risk

Concurrent Biologic Exclusion

Concurrent use of benlysta with certain other agents is excluded and may result in denial of the request.

Benlysta must not be prescribed concurrently with Lupkynis®.
Concurrent treatment with other biologics is excluded (examples include rituximab, Gazyva®, Saphnelo®).
Providers should indicate in the prior authorization whether the member is receiving any concurrent biologic or Lupkynis; concurrent use is a denial risk.

Concomitant Therapy Requirement

Benlysta must be prescribed in combination with standard therapy for SLE/LN unless all agents are contraindicated.

Concomitant standard therapy includes one or more of: glucocorticoids (e.g., prednisone), antimalarial (e.g., hydroxychloroquine or chloroquine), and non‑biologic immunosuppressants (e.g., azathioprine, methotrexate, mycophenolate, cyclophosphamide as appropriate).
Prior authorization requests must document the concomitant agents being used or state why such agents are contraindicated.
