Defines medical necessity criteria for gender-affirming medical and surgical treatment (GAMST) including eligibility, diagnostic criteria for gender dysphoria/incongruence, age-specific requirements, procedure-specific additional criteria (including facial and voice procedures), reconstructive and revision surgery coverage, and coding implications. Applies to Health Net of California members/enrollees, with note that intersex individuals are not subject to these criteria.
