Coverage criteria
Covered when ALL of the following are met:
Items are covered only when they are identified as reasonable and necessary in the member's individualized housing support plan and when the member is unable to meet the expense. All covered items must be specifically listed in that individualized housing support plan; items not listed there should not be assumed covered.
Services or items that are not identified as reasonable and necessary in the member's individualized housing support plan, or items the member is able to pay for, are excluded from coverage under this policy.
