Covered when ALL of the following are met

Eligibility: Member resides in a home (owned, rented, leased, or occupied) and is at risk for institutionalization in a nursing facility; modifications are necessary to ensure health, welfare, safety, or enable greater independence to avoid institutionalization.

Documentation: Provider order from the member's primary care physician or other qualified health professional plus a physical or occupational therapy evaluation and report documenting medical necessity (unless the managed care plan approves without an evaluation); bids or cost estimates with itemized services, costs, labor, and warranties (ideally a minimum of two bids when possible); and home visit documentation. If the residence is not owned by the member, written homeowner consent is required and documentation that modifications are permanent and the state is not responsible for maintenance.

Provider and Construction Requirements: Physical adaptations must be performed by appropriately qualified providers (contractor credential requirements apply per state rules) and Community Support (CS) providers must have relevant experience to coordinate the work.

Timing and Limits: Initial assessment and commencement of modifications must occur within 90 days of authorization initiation unless additional time is needed (for example to obtain homeowner consent or at the member's request); services are covered up to a lifetime maximum of $7,500, with exceptions considered if the member's residence or clinical needs change significantly.$7,500 lifetime maximum

Reauthorization requires submission of a new authorization with verification of work performed and reason for extension.

Adaptations or improvements that are of general utility to the household are excluded. This includes work that primarily benefits the residence rather than addressing the member’s documented medical needs. Also excluded are modifications that add to the total square footage of the home, except when the added space is necessary to complete an adaptation (for example, to improve entrance/egress or configure a bathroom to accommodate a wheelchair). Finally, finishing beyond necessary restoration to a habitable condition is limited to what is required to return the home to a usable state and does not include purely aesthetic enhancements.

EAAs that are not of direct medical or remedial benefit to the member are considered not medically necessary and are not covered. Work intended primarily for household convenience, general utility, or cosmetic purposes (such as aesthetic embellishments) does not meet medical necessity requirements. When an alternative state plan service (for example, Durable Medical Equipment) would achieve the same therapeutic goals, that service should be used instead of approving an EAA.

Prior Authorization

Authorization and reauthorization — initial authorization

Initial authorization: Community Support (CS) provider has 90 days to conduct the initial assessment and begin modifications. The service is covered until the lifetime maximum of $7,500 is met. Reauthorization: If a service extension is needed, the provider must submit a new authorization. Required information includes verification of the work performed to date and the reason for the extension (for example, more time is required to receive documentation of homeowner consent, or the individual receiving the service requests a longer time frame).

Required documentation includes a provider order and a PT/OT evaluation and report to evaluate medical necessity unless the managed care plan determines an evaluation is not needed. Bids or cost estimates (including itemized services, cost, labor, and applicable warranties) should be provided; if possible include a minimum of two bids. A home visit to determine suitability of the requested equipment or service is required.

Contractor credential and consent documentation

All environmental accessibility adaptations (EAAs) that are physical adaptations to a residence must be performed by an individual holding a California Contractor's License, except for PERS installations which may be performed per the system's installation requirements. Providers must also provide the owner and member with written documentation stating that the modifications are permanent and that the state is not responsible for maintenance, repair, or removal if the member ceases to reside at the residence.

Not Covered

The policy does not cover finishing or improvements that go beyond what is necessary to restore the home to a habitable condition when completing an adaptation. Aesthetic embellishments and other cosmetic upgrades are excluded. Improvements that represent general household utility rather than addressing a member’s medical needs are not covered, and adaptations that increase the home’s total square footage are excluded except when additional space is required to complete a medically necessary adaptation.
