Initial Therapy — Atopic Dermatitis or Vitiligo

Covered when ALL of the following are met

A. Atopic Dermatitis or Vitiligo: 1. Diagnosis of atopic dermatitis or vitiligo; 2. If request is for tacrolimus 0.03% ointment or pimecrolimus, member is ≥ 2 years of age; 3. If request is for tacrolimus 0.1% ointment, member is ≥ 16 years of age; 4. If request is for brand Protopic or Elidel, member must use generic topical tacrolimus or pimecrolimus unless clinically significant adverse effects are experienced or both are contraindicated; 5. Member meets one of the following: a) Children and adolescents: failure of 2 medium potency topical corticosteroids in the previous 6 months unless all are contraindicated or clinically significant adverse effects are experienced; b) Adults: failure of 2 high or very high potency topical corticosteroids in the previous 6 months unless all are contraindicated or clinically significant adverse effects are experienced; c) Use on the face or skinfolds; 6. B. Plaque Psoriasis (off-label): 1. Diagnosis of plaque psoriasis; 2. Age ≥ 2 years; 3. Prescribed for use on the face or intertriginous areas (e.g., genitals, armpits, forearms, groin); 4. If request is for brand Protopic or Elidel, member must use generic topical tacrolimus or pimecrolimus unless clinically significant adverse effects are experienced or both are contraindicated; 5. Initial therapy / off-label supported indications

Covered when ALL of the following are met

Primary coverage criteria: Member has a clinically appropriate indication (e.g., atopic dermatitis; off-label plaque or inverse psoriasis supported by AAD-NPF guidance for limited durations), AND there is documentation of failure, contraindication, or intolerance to standard interventions including low- to mid-potency topical corticosteroids, AND prescribed product, strength, and dosing are consistent with labeled dosing (thin layer BID) and plan quantity limits (typically up to 30 g per month or Health Plan–approved PDL quantity limits).

Off-label plaque psoriasis use added per AAD-NPF; pediatric face/genital psoriasis supported for tacrolimus 0.1% as per guidelines.

Safety exclusions: Continuous long-term use should be avoided; limit application to areas of involvement; not indicated for children <2 years; only Protopic 0.03% is indicated for ages 2–15 years.

Non‑FDA approved indications that are not specifically addressed in this policy are not authorized unless the provider supplies sufficient documentation of efficacy and safety consistent with the applicable off‑label use policy for the member’s line of business (see CP.CPA.09 for Commercial, HIM.PA.154 for Health Insurance Marketplace, and CP.PMN.53 for Medicaid) or other evidence of coverage documents.

Use of topical immunomodulators is contraindicated in patients with known hypersensitivity to the active ingredient or to any component of the product. Requests for members with documented hypersensitivity are not eligible for coverage.

Requests for off‑label use that lack sufficient clinical documentation to meet the applicable off‑label use policy are not authorized. Providers must follow and reference the plan’s off‑label use policies (e.g., CP.CPA.09, HIM.PA.154, CP.PMN.53) and submit supporting clinical evidence when requesting coverage for non‑labeled indications.

Continuous long‑term uninterrupted use of topical calcineurin inhibitors is discouraged and may be considered not medically necessary because long‑term safety has not been established. The FDA recommends avoiding continuous long‑term use in any age group and limiting application to involved areas; requests for chronic continual use should include justification and may be denied based on safety concerns.
