Clinical Policy: Tazarotene (Arazlo, Fabior, Tazorac)
Medical necessity and prior authorization criteria for topical tazarotene products (Arazlo lotion, Fabior foam, Tazorac cream/gel) for treatment of plaque psoriasis and acne vulgaris across applicable Centene lines of business.
For Illinois HIM requests, step therapy requirements do not apply to tazarotene lotion and foam as of 1/1/2026 per IL HB 5395.
Added requirement that members must use generic formulary topical tazarotene for plaque psoriasis and acne vulgaris unless clinically significant adverse effects or contraindications exist.
Clarified that prior authorization may be required for topical tretinoin and adapalene in step therapy for acne.
