Initial Therapy (Hypertension)

Covered when ALL of the following are met:

Hypertension initial approval: Member has a diagnosis of hypertension; age ≥ 18 years; failure of a combination of 3 formulary antihypertensive agents, each from different classes, used at up to maximally indicated doses unless clinically significant adverse effects occur or all are contraindicated.N/A

Approval duration: 6 months. For Illinois HIM requests, step therapy requirements do not apply as of 1/1/2026 per IL HB 5395.

Continued Therapy

Covered when ALL of the following are met:

Continued therapy: Member meets one of the following: (a) currently receiving medication via the Centene benefit or previously met initial approval criteria, or (b) currently receiving medication and is enrolled in a state/product with continuity of care regulations; AND member is responding positively to therapy.N/A

Approval duration: 12 months.

Requests for mecamylamine (Vecamyl) for indications that are not FDA‑approved and not specifically addressed in this policy are not authorized unless the provider submits sufficient documentation of efficacy and safety in accordance with the applicable off‑label use policies: CP.CPA.09 for Commercial, HIM.PA.154 for Health Insurance Marketplace, or CP.PMN.53 for Medicaid, or unless the member’s evidence of coverage documents permit the use.

Mecamylamine is contraindicated in the presence of certain comorbid conditions and concomitant therapies. Documented contraindications include concomitant use with antibiotics or sulfonamides, coronary insufficiency, glaucoma, mild or moderate hypertension, organic pyloric stenosis, recent myocardial infarction, renal insufficiency, uremia, and known hypersensitivity to mecamylamine.
