Coverage Criteria for Liraglutide (Saxenda)

Initial Therapy

Covered when ALL of the following are met:

Eligibility and baseline requirements: Member meets applicable BMI and age criteria; documentation of current height and weight within the last 30 days to calculate BMI; member is actively participating in a Health Net–approved or prescriber‑recommended weight loss program involving reduced calorie diet, increased physical activity, and behavioral modification for at least 6 months and will continue participation while on therapy; liraglutide (Saxenda) is not prescribed concurrently with other liraglutide‑containing products or any other GLP‑1 receptor agonist(s); for Saxenda requests, member must use generic liraglutide unless contraindicated or clinically significant adverse effects are experienced.see BMI and enrollment requirements

Prior authorization may be required; medical justification required when switching from another GLP‑1 to liraglutide (Saxenda).

BMI/Age branching

Adult criteria: BMI ≥ 30 kg/m2 OR BMI ≥ 27 kg/m2 with at least one weight‑related comorbidity (e.g., coronary artery disease, hypertension, dyslipidemia, diabetes, elevated waist circumference, sleep apnea); age ≥ 18 years for members with concurrent T2DM; age ≥ 12 years for members without concurrent T2DM.

T2DM‑specific step therapy: For members with concurrent type 2 diabetes mellitus (T2DM): failure of ≥3 consecutive months each of the following agents is required unless clinically significant adverse effects occur or agents are contraindicated: (i) Ozempic® or Rybelsus®, (ii) Trulicity®, (iii) liraglutide (generic Victoza®), and (iv) Mounjaro®. If an existing member is currently receiving a GLP‑1 and requests to switch to liraglutide (Saxenda), medical justification must support necessity.

Intolerance due to common GI adverse effects is not acceptable justification.

Dosing and approval duration: Dose escalation per prescribing information to a maintenance dose as tolerated; adult maintenance dose 3 mg/day (dose must not exceed 3 mg/day; ≤5 pens per month); pediatric maintenance at least 2.4 mg/day if 3 mg not tolerated; approval duration for initial therapy: 16 weeks.3 mg/day; 16 weeks

Dose escalation schedule: Week 1 0.6 mg QD, Week 2 1.2 mg QD, Week 3 1.8 mg QD, Week 4 2.4 mg QD, Week 5 onward 3 mg QD (pediatric escalation may take up to 8 weeks).

Continued Therapy

Covered on renewal when ALL of the following are met:

Continuation eligibility: Member is currently receiving the medication via the benefit or has previously met initial approval criteria; new members transitioning from another health plan are not automatically grandfathered (refer to initial criteria); members transitioning to a new benefit with higher BMI threshold may be grandfathered per plan rules unless therapy was stopped for >60 days (in which case a new authorization at the current benefit is required).

Liraglutide (Saxenda) must not be prescribed concurrently with other liraglutide‑containing products or any other GLP‑1 receptor agonist; concurrent prescriptions with another GLP‑1 will not be authorized.
