Initial Therapy Criteria — Weight Management

I. Initial Approval Criteria - Weight Management

Provider must submit documentation supporting that member has met all approval criteria:

Member meets one of the following
- BMI ≥ 30 kg/m2
- BMI ≥ 27 kg/m2 with at least one indicator of increased cardiovascular risk (e.g., coronary artery/heart disease, hypertension, dyslipidemia, diabetes, elevated waist circumference) or other obesity-related medical condition (e.g., sleep apnea)
- If age is between 12 and 17 years: BMI ≥ 95th percentile standardized for age and sex

Weight management reauthorization must meet all criteria below:

Clinical Evidence & Definitions

Orlistat is a reversible inhibitor of gastrointestinal lipases indicated for obesity management, including weight loss and weight maintenance when used with a reduced-calorie diet, and to reduce risk of weight regain after prior weight loss. Capsule strength is 120 mg and recommended dosing is 120 mg PO TID with each main meal containing fat (maximum 360 mg/day).
