Pathological examination of tissue obtained from a prostate saturation biopsy does not meet coverage criteria for the diagnosis, staging, or management of prostate cancer. The policy defines saturation biopsy as extensive sampling (up to 24 cores) and explicitly excludes these specimens from coverage due to insufficient published evidence demonstrating required clinical benefit. Providers should expect potential denial of claims for pathological analysis of saturation biopsy specimens unless alternative coverage criteria apply.

The National Comprehensive Cancer Network (NCCN) does not support routine anteriorly directed sampling at initial biopsy and states that saturation biopsy is not recommended routinely for all individuals with previous negative biopsies, given the demonstrated benefits of pre-biopsy multiparametric MRI and MRI-targeted biopsy techniques. The NCCN recommends considering MRI followed by targeted biopsy when clinical suspicion persists after a negative biopsy and prefers a combined systematic plus targeted approach when MRI-targeting capabilities are available, particularly at initial biopsy.

Saturation biopsy specimen analysis is considered not medically necessary because available published scientific literature does not confirm that the test is required and beneficial for diagnosis or treatment. The policy therefore does not support coverage for pathological examination of saturation biopsy cores.

The NCCN panel specifically states that it does not recommend a saturation biopsy strategy for all individuals with prior negative biopsies at this time. Instead, the panel endorses consideration of mpMRI and MRI-targeted biopsy for persistent clinical suspicion, reserving saturation approaches for selected circumstances rather than routine use.

Clinical triggers to pursue biopsy include an abnormal or rising PSA, a palpable nodule on digital rectal examination (DRE), or suspicious radiologic findings. Initial diagnostic sampling that meets coverage uses a 12-core extended sampling scheme; repeat biopsy may be appropriate when clinical suspicion persists despite a prior negative biopsy (excluding saturation biopsy). Multiparametric MRI (mpMRI) and targeted biopsy strategies can improve detection of clinically significant cancers and inform decisions about repeat sampling.

Specimen handling must follow the policy note: submit one vial per sextant with no more than two cores per vial. For billing, each vial — regardless of the number of cores contained (up to the allowed two cores per vial) — is considered a single specimen. Bill accordingly and document specimen labeling and vial counts in the pathology report and claim documentation.
