inv-03: HPV tumor status testing (CC1)

Covered when testing is to establish HPV tumor status for specific clinical indications:

CC1 expanded indication: Testing for high-risk HPV with either mRNA expression testing for HPV E6/E7 or immunohistochemistry for p16 expression meets coverage criteria for individuals with oropharyngeal squamous cell carcinoma OR with metastatic squamous cell carcinoma of unknown primary origin in a cervical lymph node.

inv-04: Accepted HPV detection methods (CC2)

Covered detection modalities (examples):

CC2.e: Detection of HPV from an oropharyngeal swab (for example, OmniPathology Oropharyngeal HPV PCR Test) is an accepted detection method.

CC2 - surrogate and molecular methods: p16 immunohistochemistry (IHC) as a surrogate marker and HPV E6/E7 mRNA expression testing are accepted methods to establish HPV tumor status.

p16 IHC recommended by CAP and endorsed by NCCN/ASCO; E6/E7 mRNA considered the gold standard.

Routine screening of asymptomatic individuals for oral cancer using oropharyngeal swabs, salivary HPV/genotyping tests, or other salivary biomarker assays is not supported by this policy and does not meet coverage criteria. The policy explicitly lists salivary biomarker testing (peptides/proteins, nucleic acids, metabolites), HPV genotyping (e.g., OraRisk®), gene expression profiling, mixed panels that combine genetic and nongenetic biomarkers (e.g., mRNA CancerDetect™), and detection of HPV from oropharyngeal swabs as tests that do not meet coverage for screening, detection, or diagnosis of oral cancer.

The rationale is that there is a lack of sufficient published scientific literature demonstrating these tests are required and beneficial for diagnosing or managing an individual’s oral cancer; authoritative guidelines (including the College of American Pathologists) do not recommend routine swab or salivary screening in asymptomatic patients and endorse testing tied to confirmed or suspected tumors, nodal metastases, histology, or specific staging/prognostic/therapeutic decisions.

The USPSTF concluded that the evidence available as of its 2013 recommendation was insufficient to assess the balance of benefits and harms of screening for oral cancer in asymptomatic adults. The USPSTF also noted that, despite interest in screening for oral HPV infection, medical and dental organizations do not recommend routine screening for oral HPV.

Because of this insufficiency of evidence, routine population-based screening for oral cancer or oral HPV infection is not supported by the USPSTF and is reflected in the policy’s exclusion of salivary and swab-based screening tests from coverage.

Use of salivary biomarker assays, commercial HPV genotyping panels such as OraRisk® (which reports multiple HPV genotypes), gene expression profiling assays (for example, mRNA CancerDetect™), combined panels that mix genetic and nongenetic biomarkers, and detection of HPV from oropharyngeal swabs are explicitly listed as not meeting coverage criteria for screening, detection, or diagnosis of oral cancer.

The policy identifies specific commercial tests and formats as examples of non-covered technologies because current published evidence does not demonstrate they are required or beneficial for clinical management of oral/oropharyngeal cancer.

Consistent with guideline conclusions, routine screening for oral cancer and routine screening for oral HPV infection in asymptomatic adults is not recommended. The policy cites evidence and guideline statements noting that routine swab-based or salivary screening of asymptomatic individuals is not endorsed (for example, CAP guidance and USPSTF findings).

As a result, population-level or routine screening with oropharyngeal swabs or salivary HPV/genotyping tests is not supported by the USPSTF and is not covered under this policy.

Background and Rationale

Oral cancer (most commonly squamous cell carcinoma) arises in the oral cavity and oropharynx; early detection of potentially malignant disorders can prevent malignant transformation. Human papillomavirus (HPV), particularly high-risk types, is an important contributor to oropharyngeal cancers, and assessing tumor HPV status has diagnostic, prognostic, and therapeutic relevance in confirmed or suspected tumors.

However, there is no approved screening test for oral or oropharyngeal HPV infection in asymptomatic individuals, and adjunctive or salivary tests (vital staining, autofluorescence, brush cytology, salivary biomarkers, commercial genotyping, and gene expression profiling) have been studied but have not been shown to replace a conventional oral examination followed by biopsy and histologic confirmation when indicated. Therefore, testing for HPV tumor status is covered only when performed to establish HPV status in tissue-confirmed oropharyngeal squamous cell carcinoma or metastatic SCC of unknown primary in a cervical lymph node using validated methods (p16 IHC or E6/E7 mRNA), and routine screening with salivary or swab-based assays is not supported.
