ENFD for diagnosis of small‑fiber neuropathy

Covered when ALL of the following are met:

ENFD coverage criteria: 1) Individual presents with symptoms of painful sensory neuropathy; 2) No history of a disorder known to predispose to painful neuropathy (e.g., diabetic neuropathy, toxic neuropathy, HIV neuropathy, celiac neuropathy, inherited neuropathy); 3) Physical examination shows no evidence of findings consistent with large‑fiber neuropathy (eg, reduced/absent reflexes or reduced proprioception/vibration); 4) Electromyography and nerve‑conduction studies are normal and show no evidence of large‑fiber neuropathy.all

Epidermal/epidermal nerve fiber density (ENFD/IENFD) measurement by skin biopsy meets coverage criteria only when all specified diagnostic conditions are documented. ENFD testing does not meet coverage criteria for situations outside the defined diagnostic scenario. Measurement of sweat gland nerve fiber density (SGNF/SGNFD) is explicitly not covered.

An international expert panel on Fabry disease states that, given availability of accurate diagnostic laboratory testing, nerve or skin biopsy is not required to diagnose Fabry disease. Skin biopsy may detect small‑fiber involvement and quantify loss of innervation, but it is not necessary to establish the diagnosis when validated laboratory tests are available.

For Fabry disease, the policy reiterates that nerve or skin biopsies are not required for diagnosis when accurate diagnostic laboratory tests exist. Skin biopsy can be used to document small‑fiber involvement or quantify nerve loss, but routine biopsy solely to confirm Fabry disease is not indicated.

Skin biopsy ENFD testing has limited utility in conditions where functional testing is the primary diagnostic modality. For example, evidence indicates ENFD measurement is not generally useful for diagnosing erythromelalgia, where functional nerve testing more reliably identifies the condition.

Published retrospective data in erythromelalgia show that most patients have abnormalities on functional nerve testing while fewer than 10% demonstrate decreased ENFD; accordingly, skin biopsy has limited diagnostic utility for erythromelalgia and is often unnecessary.

After the literature review completed for the 09/04/2024 update, there were no modifications to the policy's not medically necessary statements.

Bedside and Electrophysiologic Testing Recommended Before Biopsy

Before skin biopsy, perform simpler, noninvasive bedside tests and nerve conduction studies when appropriate. Skin biopsy (ENFD) is generally indicated when bedside testing and NCS/EMG are non-diagnostic and clinical suspicion for small-fiber neuropathy remains.

Recommend 10-g monofilament and 128-Hz tuning fork, temperature or pinprick testing, and reflex assessment prior to biopsy when applicable.
Nerve conduction studies / EMG should be normal (no evidence of large-fiber neuropathy) to support ENFD testing for small-fiber neuropathy.

Note

Limited Utility of Nerve Biopsy for DSP

Nerve biopsy has limited utility in distal symmetric polyneuropathy (DSP). Guidelines note insufficient evidence to recommend routine nerve biopsy for DSP; nerve biopsy may be useful in select atypical neuropathies (e.g., suspected vasculitis, amyloid, mononeuritis multiplex) but not for routine evaluation of DSP.

Nerve biopsy: generally not recommended for routine evaluation of DSP (Level U evidence per specialty guidance).
Consider nerve biopsy only for atypical presentations where etiology cannot be determined by less invasive testing.
