Organism-specific coverage table

Coverage status is provided per organism and test type; 'MCC' indicates meets coverage criteria, 'DNMCC' indicates does not meet coverage criteria.

Organism/Test-Level Coverage: Each organism and test type (Direct Probe, Amplified Probe, Quantification) is assigned a coverage status: MCC = meets coverage criteria; DNMCC = does not meet coverage criteria. See policy table entries for specifics.

Examples from the policy table: Chlamydia pneumoniae — Direct Probe (87485) = DNMCC; Amplified Probe (87486) = MCC. Cytomegalovirus — Direct Probe (87495) = DNMCC; Amplified Probe (87496) = MCC; Quantification (87497) = MCC.

Revised coverage exclusions (06/04/2025)

Changes and exclusions applied in the June 4, 2025 revision

Direct probe exclusion: Direct probe testing for Chlamydia pneumoniae, Cytomegalovirus, Legionella pneumophila, and Mycoplasma pneumoniae do not meet coverage criteria (DNMCC).

Applied to direct probe CPT codes listed in the policy table (see revision notes).

Simultaneous ordering exclusion: Simultaneous ordering of amplified probe and quantification for the same organism in a single encounter does not meet coverage criteria.

Prevents same-day amplified probe + quantification for identical target.

Removal of specific CPTs: CPT codes 87480, 87481, and 87482 were removed from the policy listings.

Administrative/code list change documented in revision history.

Simultaneous ordering of an amplified probe test (NAAT/ amplified probe technique) and a separate quantification test for the same organism during the same clinical encounter does not meet coverage criteria. Orders submitted with both an amplified probe and a quantification code for the identical target may be denied; verify ordering against the organism-specific table before submission.

The June 4, 2025 revision recategorized direct probe methods so that all direct probes do not meet coverage criteria (DNMCC) and reorganized policy language to expressly prohibit same‑day ordering of amplified probe plus quantification for the same organism. This change moved direct probe entries out of covered categories and clarified that simultaneous amplified probe + quantification in a single encounter does not meet coverage criteria.

Any test entry in the organism coverage table labeled DNMCC (Does Not Meet Coverage Criteria) is considered not covered under this policy. This includes direct probe techniques and quantification codes explicitly marked DNMCC; such tests should not be ordered when the intent is to obtain covered services under the member’s benefit. Providers should confirm the table entry for the specific CPT/HCPCS code prior to ordering.

Direct probe testing for Chlamydia pneumoniae, Cytomegalovirus, Legionella pneumophila, and Mycoplasma pneumoniae has been recategorized to DNMCC and therefore does not meet coverage criteria under the updated policy. Amplified probe (NAAT) methods for these organisms may retain MCC status where indicated in the organism table; refer to the code-specific entries for permitted tests.

Clinical suspicion indications

Covered when clinical suspicion aligns with public-health or specialty guidance recommending NAAT/PCR as the preferred diagnostic modality.

Mpox (orthopoxvirus): Suspected mpox (probable or suspect cases per CDC): collect lesion swabs and perform orthopoxvirus or MPXV-specific PCR (real-time PCR is diagnostic); two swabs per lesion are recommended for testing and downstream clade-specific analysis.

CDC defines suspect/probable/confirmed cases and recommends PCR for diagnosis.

Chlamydia pneumoniae: Acute C. pneumoniae infection when clinical presentation warrants testing: NAATs/real-time PCR are the preferred diagnostic tests due to high sensitivity and timeliness for treatment decisions.

Follow CDC guidance recommending NAAT/PCR.

Clostridioides difficile (C. difficile): When there is clinical suspicion of C. difficile infection use FDA-approved PCR assays (amplified probe) for detection of toxin genes as they are highly sensitive and specific; interpret results in clinical context due to possible colonization.

CDC describes PCR as same-day, highly sensitive tests but cautions on interpretation with multiplex panels.

Public health / specialized lab testing indications

Public health and specialized laboratory testing indications where molecular methods (rRT-PCR, sequencing) or confirmatory sequencing are recommended.

Non-polio enteroviruses / Parechoviruses: Detection performed by public-health laboratories using rRT-PCR or molecular sequencing for enteroviruses, parechoviruses, and uncommon picornaviruses when clinically indicated (e.g., CNS infections).

CDC notes routine qualitative testing and use of rRT-PCR/sequencing in public health labs.

Mpox (orthopoxvirus) — public health confirmation: Probable or suspected mpox cases should be tested using MPXV-specific PCR or orthopoxvirus PCR with confirmation by sequencing, per ECDC/UKHSA and CDC guidance.

ECDC recommends sequencing confirmation for orthopoxvirus PCR-positive probable cases.

Quantitative HHV-6 testing criteria

Quantitative HHV-6 testing is used to distinguish active infection from latent/persistent detection when viral load exceeds published thresholds.

HHV-6 quantitative testing criteria: Quantitative PCR testing for HHV-6 is covered when used to differentiate active versus latent infection and interpretation relies on viral load thresholds; active infection is suggested when viral load is >200 copies/mL or >20 copies per microgram of DNA.>200 copies/mL or 20 copies/μmg DNA

HHV-6 Foundation guidance: qualitative PCR on whole blood is insufficient to distinguish active from latent infection; use quantitative testing.

Not Covered

Tests or test types shown as DNMCC in the organism table are not covered; this includes the direct probe technique entries and specific quantification codes listed as DNMCC. Interpret DNMCC exactly as an exclusion from coverage under this policy and do not substitute these tests when attempting to meet coverage requirements for the same clinical indication.

Direct probe tests for Chlamydia pneumoniae, Cytomegalovirus, Legionella pneumophila, and Mycoplasma pneumoniae are explicitly categorized as DNMCC in the updated policy and are not covered. Providers should order alternative validated amplified probe (NAAT) or quantitative tests only when those tests are designated MCC for the organism in the policy table and consistent with member benefits.

Covered Indications

Clinical suspicion of infection by specified organisms where NAAT/PCR is preferred

Coverage applies when clinical suspicion and testing follow CDC/IDSA/WHO guidance indicating NAAT/PCR as the preferred diagnostic modality for the listed organisms.

Organism-specific covered indications: Covered clinical indications include suspected infections where public health or specialty guidance recommends NAAT/PCR testing: mpox (orthopoxvirus), congenital CMV, C. difficile infection, C. pneumoniae, MRSA carriage/infection, M. pneumoniae, enterovirus/parechovirus, RSV, and others as cited in guidance.

Detection of non-polio enteroviruses, parechoviruses, and mpox testing

Public health and reference laboratory testing methods and confirmation pathways for specific viral pathogens.

Enterovirus / Parechovirus detection: Detection of non-polio enteroviruses, parechoviruses, and other picornaviruses is performed by public health laboratories using rRT-PCR or sequencing; these methods are recommended for suspected CNS and severe systemic infections.

Background

Quantitative PCR (qPCR) extends these methods by providing viral/bacterial load information that can help distinguish latent carriage from active infection for select organisms. While these molecular methods offer clinical advantages, they may carry risks of contamination, variability between assays, and interpretation complexity; use organism-specific guidance in the policy (and public health/clinical society guidance) to select the appropriate test modality and specimen type.
