Defines coverage criteria for laboratory tests used to diagnose active tuberculosis (microbiologic tests) and to screen or diagnose latent TB infection (IGRAs and TST), and lists tests that do or do not meet coverage criteria for members of Medical Mutual - Ohio.
Direct probe testing is no longer available; qualitative NAAT testing becomes CC2.c for Mycobacteria spp., M. tuberculosis, and M. avium complex.
Removed CPT codes 87149, 87550, 87555, 87560 from the policy.
