Laboratory Testing for the Diagnosis of Inflammatory Bowel Disease
Defines coverage for fecal and serologic laboratory tests used to diagnose or monitor inflammatory bowel disease (IBD) including ulcerative colitis and Crohn's disease for Medical Mutual - Ohio members.
Fecal calprotectin or fecal lactoferrin testing MEETS COVERAGE CRITERIA for: a) differentiating non-inflammatory GI disease from inflammatory GI disease; b) monitoring individuals with IBD (assess response to therapy or relapse).
For all other situations not described above, fecal calprotectin and fecal lactoferrin testing DOES NOT MEET COVERAGE CRITERIA.
Former CC2 was renumbered to CC4 and edited: multianalyte serum biomarker panels (with or without algorithmic analysis) designed to distinguish IBD from non-IBD or to diagnose/monitor IBD DO NOT MEET COVERAGE CRITERIA (examples: ibs-smart™, IBSchek®, PredictSURE IBD™, Prometheus®).
Added CPT codes 83630 and 83993 and CPT II code 0598U (effective 10/1/2025).
New Note 1: Fecal calprotectin is the preferred biomarker; if both fecal calprotectin and fecal lactoferrin are ordered together, only fecal calprotectin will be approved.
