Cardiovascular Disease Risk Assessment
Rules for coverage and limitations for laboratory and biomarker testing used to assess cardiovascular disease (CVD) risk in adults, including lipid panels, apoB, Lp(a), hs-CRP, and other biomarkers; applies to members whose benefits are governed by this payer policy.
Lp(a) screening is once per lifetime and should occur at age 18 years or older.
hs-CRP screening requires one initial test, a confirmatory test no sooner than two weeks after an abnormal initial result, and annual testing thereafter if elevated CRP is confirmed.
Policy defines which CRP assays do NOT meet coverage criteria: hs-CRP for other unspecified CVD risk assessments and conventional CRP for CVD risk assessment.
