Overall Coverage Stance

Existing coverage criteria remain in effect. Formatting was adjusted so that entries which MEET COVERAGE CRITERIA are listed before those that DO NOT MEET COVERAGE CRITERIA. No new exclusions were added in this revision; the literature review did not necessitate modifications to coverage criteria.

The policy continues to endorse quantitative cardiac troponin (I or T) as the preferred biomarker for diagnosis of myocardial infarction when measured in outpatient facilities capable of performing an adequate clinical MI evaluation, and in alignment with guideline-recommended timing and serial sampling.

BNP and NT-proBNP measurements are supported for diagnosis of heart failure in patients presenting with dyspnea and for establishing disease severity or prognosis in chronic heart failure, consistent with guideline recommendations; serial measurements for routine therapeutic guidance remain unsupported by conclusive evidence for improving mortality or reducing hospitalizations.
