Replacement Lower Limb Prostheses

Covered when ALL of the following are met per InterQual (customized) replacement criteria and plan benefit:

The following devices and device features are excluded from coverage when they do not reasonably improve or stabilize the member's condition or functioning. Specifically, items are excluded when they are unlikely to improve or stabilize the member's condition or are suboptimal compared with other acceptable options that can achieve the same clinical outcome. Coverage is also excluded for devices that are principally for convenience or cosmetic purposes (for example, swim or shower prostheses and cosmetic-only artificial limbs or parts), devices that are primarily recreational or avocational, or devices considered experimental or investigational under the plan's experimental and investigational policy.

Requests for prosthetic devices that do not demonstrably improve function, or where a more appropriate, less intensive model adequately meets the member's medical needs and activities of daily living, are not considered medically necessary. Devices and DME features intended mainly for convenience, cosmetic enhancement, recreational use, or those classified as experimental/investigational per plan policy will be denied as not medically necessary.

Medicare Functional Classification Level (MFCL) — Levels 0–4

MFCL Level 0Does not have the ability or potential to ambulate or transfer safely with or without assistance; a prosthesis does not enhance quality of life or mobility.

MFCL Level 1Has ability or potential to use a prosthesis for transfers or ambulation on level surfaces at a fixed cadence (typical limited or unlimited household ambulator).

MFCL Level 2Has ability or potential for ambulation with ability to traverse low-level environmental barriers (curbs, stairs, uneven surfaces); typical limited community ambulator.

MFCL Level 3Has ability or potential for ambulation with variable cadence; can traverse most environmental barriers and may engage in vocational, therapeutic, or exercise activities demanding prosthetic use beyond simple locomotion.

Documentation Required

Required Clinical Documentation and Provider Qualifications

Treating physical medicine & rehabilitation (PM&R) physicians must work with a contracted Mass General Brigham Health Plan prosthetics provider. The prosthetics provider must have a prosthetist on staff currently certified by the American Board for Certification in Orthotics, Prosthetics, and Pedorthics (ABC) or the Board of Certification/Accreditation, International (BOC). That prosthetist evaluates the member and coordinates with the treating provider to gather and submit all necessary clinical documentation for benefit and medical necessity review.
