Clinical documentation without systemic condition: If no systemic condition is present, documentation must show clinical evidence of mycosis with 3 of 5 specific findings, the extent/manner of debridement, and qualifying symptoms (marked limitation in ambulation, significant pain beyond 'painful nails', or secondary infection).3 of 5 findings

Required findingsClinical evidence of mycosis requires at least 3 of the following 5: nail hypertrophy/thickening; lysis or loosening of the nail plate; discoloration; brittleness; subungual debris.

Required Clinical Documentation — For mycotic nails absent a systemic condition: document the affected toe(s); clinical evidence of mycosis (at least 3 of the following: nail hypertrophy/thickening; lysis or loosening of the nail plate; discoloration; brittleness; and/or subungual debris); the manner and extent of debridement performed; and the antifungal agent used if applicable. Document qualifying symptoms such as marked limitation in ambulation, pain beyond nonspecific 'painful nails', or secondary infection resulting from nail thickening/dystrophy. Based on presence of a systemic condition: include the diagnosis code of the covered indication and the systemic condition diagnosis code; report one Class A finding with modifier Q7 (non‑traumatic amputation of foot or integral skeletal portion thereof) OR two Class B findings with modifier Q8 (e.g., absent posterior tibial pulse, absent dorsalis pedis pulse, advanced trophic changes — where three of the listed minor findings equal one Class B finding such as absent/decreased hair growth or nail changes with thickening). Alternatively, document one Class B and two Class C findings with modifier Q9 (e.g., claudication, temperature changes, edema, paresthesias, burning). Include name and NPI of the referring/ordering physician actively treating the complicating condition and the date last seen (actively treating = seen for treatment or evaluation within 6 months prior to, or within 30 days after, the debridement procedure).

inv-15: Hypertrophic toenails

DefinitionEnlargement of the nails with abnormal curving (onychogryposis) or massive overgrowth of the toenails (onychauxis).

Clinical relevanceHypertrophic toenails may cause pain, secondary infection, or limit ambulation and can justify debridement when these complications are present.

TerminologySynonyms: onychogryposis (abnormal curving) and onychauxis (massive overgrowth).

inv-16: Mycotic

DefinitionInfected with fungus; may present with thickening, yellowing, loosening from the nail bed, and subungual debris.

Common signsMay show nail thickening, yellow discoloration, and potential separation (onycholysis) from the nail bed.

Documentation requirementWhen no systemic condition is present, document 3 of 5 specific mycotic findings to support debridement.

inv-17: Nail debridement

DefinitionRemoval of all diseased nail to the nail bed or viable nail plate or to the point where bleeding is imminent; simple trimming or grinding is not debridement.

What is not debridementSimple trimming or grinding of toenails (cutting/filing ends) is not considered debridement and is not reimbursable as nail debridement.

DocumentationRecord the manner in which and extent to which the nail was debrided in the office/progress note.
