Authorization may be granted for members new to the plan within past 90 days who are currently receiving the medication (excluding samples or manufacturer assistance) OR when ALL of the following are met:

Entry pathways: Member is new to plan within past 90 days and currently receiving requested medication (not obtained as samples or via manufacturer's patient assistance programs)

Excludes medication obtained as samples or through manufacturer's patient assistance programs

Tablet trial requirement: Member has had a trial and failure, intolerance, or contraindication to imatinib tablet OR a clinical rationale is provided why the member cannot administer imatinib tablet

This pathway is required when the member is not new to the plan within 90 days

Supporting documentation must be submitted with the prior authorization request. For new-to-plan members (enrolled within 90 days), include evidence the member is currently receiving the requested medication. Medications obtained as samples or via manufacturer patient assistance programs cannot be used to meet the "currently receiving" requirement.

For members new to the plan within 90 days: documentation that the member is currently receiving the requested medication (exclude samples/PAP)
