Initial hospital outpatient facility criteria

Covered in hospital outpatient facility when ONE OR MORE of the following are met (member is medically unstable for alternative care):

Hospital outpatient infusion criteria: a) Adverse event with the requested product not responding to conventional interventions (e.g., acetaminophen, steroids, diphenhydramine, fluids, other pre-medications, or slowing infusion rate) OR occurring during or immediately after an infusion (e.g., anaphylaxis, anaphylactoid reactions, myocardial infarction, thromboembolism, seizures); b) Respiratory, cardiovascular, or renal conditions that limit tolerance of large volume load or predispose to severe adverse events that cannot be managed in alternate settings without appropriate personnel/equipment; c) Severe venous access issues requiring special interventions only available in the outpatient hospital setting; d) Physical or cognitive impairments that would present an unnecessary health risk.

Home infusion criteria

Home infusion is appropriate when ALL of the following are met:

Home infusion criteria: 1) Home environment assessment: accessible to 911/urgent care; adequate refrigeration; general cleanliness per onsite home nursing assessment; 2) Member is medically stable and able to participate in their care; 3) Member has reliable venous access.

When the conditions listed in the hospital outpatient or home infusion criteria are not satisfied, the requested site-of-care authorization is not supported. Coverage is excluded when the relevant criteria above are not met or when the specialty infusion medication itself has been denied.

Authorization is required for specialty infusion medication site of care for Commercial and Connector/Qualified Health Plans. Mass General Brigham Health Plan collaborates with Optum Fusion for most specialty infusion medications. This policy applies only to members age 14 years and older and does not pertain to authorization of the medication itself; it addresses the appropriate site of care (hospital outpatient facility versus less intensive settings such as home).

Grace Periods and Authorization Continuation

The plan uses medication-specific grace periods to determine when a member can be transitioned from a hospital outpatient facility to a less intensive site of care (e.g., home). After the applicable grace period, Optum Fusion may continue to authorize infusion in the hospital outpatient facility setting only when the clinical criteria for hospital outpatient infusion are met.

Documentation Required

Required Clinical Documentation

Clinical documentation must clearly support the decision for hospital outpatient infusion versus home infusion. Documentation should indicate whether the member meets one or more of the hospital outpatient criteria (examples below), and should include relevant history, clinical findings, and any prior infusion reactions or interventions attempted. Provide sufficient detail so the reviewer can determine medical necessity for the requested site of care.

Document presence of adverse events with the requested product, including timing (during or immediately after infusion) and response to conventional interventions (e.g., acetaminophen, steroids, diphenhydramine, fluids, pre-medications, slowing infusion rate). Include diagnoses for events such as anaphylaxis, myocardial infarction, thromboembolism, seizures, or other serious reactions.
Document respiratory, cardiovascular, or renal conditions that limit tolerance of large volume load or increase risk of severe adverse events that cannot be managed outside a hospital outpatient setting.
Document severe venous access issues requiring interventions available only in the outpatient hospital setting.
Document physical or cognitive impairments that present unnecessary health risk for home infusion.
For home infusion requests, document home assessment findings: accessibility to emergency services (911/urgent care), adequate refrigeration, general cleanliness as determined by onsite home nursing assessment, member medical stability and ability to participate in care, and reliable venous access.
