General Coverage Criteria

Covered when ALL of the following are met:

General coverage criteria: 1) The member has a diagnosis of HIV or AIDS; 2) The medical condition is well documented by clinical notes (photos may be required) including a diagnosis of HIV-associated lipodystrophy syndrome and a statement that the treatment is necessary for correcting, repairing, or ameliorating the effects of HIV-associated lipodystrophy syndrome; 3) The requested procedure can be reasonably expected to treat the specific part of the body affected by HIV-associated lipodystrophy syndrome.

Facial (Sculptra, Radiesse, Autologous Fat Transplantation)

Facial procedures are covered when ALL of the following are met:

Facial interventions: Sculptra and Radiesse dermal filler injections for facial lipoatrophy due to HIV-associated lipodystrophy syndrome, and autologous fat transplantation, are medically necessary when the general coverage criteria are met and the provider performing the procedure is a contracted in-network provider.

Subsequent injections require prior authorization and clinical documentation from the treating provider.

Chest Liposuction

Chest lipohypertrophy interventions covered when ALL of the following are met:

Liposuction for chest: Liposuction to reduce chest lipohypertrophy caused by HIV-associated lipodystrophy syndrome is covered when the member meets the general coverage criteria.

Gynecomastia Surgery

Gynecomastia surgery covered when ALL of the following are met:

Gynecomastia surgery to reduce chest lipohypertrophy caused by HIV-associated lipodystrophy syndrome is covered when the member meets the general coverage criteria and when liposuction is not indicated to treat chest lipohypertrophy.

Abdomen (Tesamorelin)

Abdominal treatment covered when ALL of the following are met:

Tesamorelin (Egrifta) injections are covered for abdominal lipohypertrophy due to HIV-associated lipodystrophy syndrome when the general coverage criteria are met and when authorized through the retail pharmacy program.

Covered via the retail pharmacy benefit; pharmacy authorization required.

Neck/Upper Back Liposuction

Neck/Upper back interventions covered when ALL of the following are met:

Liposuction for neck/upper back: Liposuction to reduce lipohypertrophy of the neck (dorsocervical region) is covered when the member meets the general coverage criteria.

Covered when clinical indication and supporting evidence for safety and efficacy are present:

Covered treatments for HIV-associated lipodystrophy: Dermal fillers (e.g., calcium hydroxyapatite, poly-L-lactic acid), autologous fat transfer, drug therapy (e.g., tesamorelin), liposuction, and surgical lipectomy are covered for specific indications associated with HIV-associated lipodystrophy when those therapies have demonstrated safety and efficacy in the evidence base.

Clinical approach varies by phenotype:

Central fat accumulation: For central lipohypertrophy (eg, dorsocervical 'buffalo hump', abdominal visceral fat), treatment options include liposuction and excisional lipectomy; retrospective data suggest excisional lipectomy may have lower recurrence and reoperation rates compared with liposuction in some analyses.

In addition, semipermanent dermal fillers or autologous fat transplantation used for any indication other than facial lipoatrophy due to HIV-associated lipodystrophy syndrome are excluded. These exclusions align with the policy's general exclusions for cosmetic procedures when the member does not meet the general coverage criteria or when the procedure is solely to enhance appearance without functional abnormality.
