Routine administration of oral medications or general maintenance of ostomies are considered unskilled and not covered as skilled services unless the member's condition makes licensed care necessary.

Discipline‑specific examples should be documented to show skilled need

Mass General Brigham Health Plan does not cover unskilled care. Services that are routine or that can be safely and effectively provided by non‑licensed caregivers are excluded from coverage.

Physical Therapy — Unskilled examples: general fitness exercises, repetitive services to maintain function after a maintenance program is established, passive ROM or chronic stretching when maintenance only, programs not intended to restore lost function or without expected improvement in a reasonable period.
Occupational Therapy — Skilled examples: customized ADL treatment programs, comprehensive home evaluations with adaptation recommendations, performance skills assessment and treatment, assessment/fabrication/training for adaptive equipment and orthotics, teaching compensatory techniques to improve independence.
Speech‑Language Pathology — Skilled examples: evaluation and treatment of speech, language, and swallowing disorders (e.g., dysphagia, aphasia, apraxia, dysarthria), diagnostic services to determine type/causation/severity, therapeutic services for medical disorders causing communication/swallowing deficits.
