Prior authorization is required for Kymriah. Commercial and Qualified Health Plan requests are managed by Prime Therapeutics (follow Prime Therapeutics Kymriah policy). Mass General Brigham ACO members require prior authorization via the MassHealth Drug Utilization Review Program (see MassHealth Table 75). Senior Care Options (SCO) members also require authorization. Adult Large B-cell Lymphomas and Follicular Lymphoma (patients >= 18 years)

Required clinical documentation
