Coverage Criteria for Oral & Maxillofacial Procedures

Cleft lip and palate

Covered when the following age- and authorization-based conditions are met

Cleft repairs in children: Primary and secondary cleft lip and cleft palate repair, reconstruction, and excision for members under age 18 do not require prior authorization and are covered as medically necessary when related to treatment of cleft lip/palate.age <18

Includes related services such as plastic surgery, speech therapy, audiology, nutrition, preventative and restorative dentistry, and orthodontic treatment related to cleft care for commercial members under 18.

Adult cleft repairs: For members 18 years of age and older, prior authorization is required for cleft lip, palate, and nasolabial repair.age >=18

Orthognathic and airway surgery

Covered when medical necessity criteria (including imaging and prior therapy) are met

Orthognathic surgery for correction of a significant skeletal abnormality is covered when medical necessity is met as determined by InterQual criteria.InterQual

Maxillomandibular/mandibular advancement for OSA: Covered when one of the following is met: moderate or severe OSA (AHI/RDI >= 15) OR mild OSA (AHI/RDI 5-14) with significant oxygen desaturations and/or Epworth >9; and there is documentation of failure of PAP titration or adherence despite coaching and adjustments, or failure/intolerance of an oral appliance. The requested surgical procedure must be expected to significantly improve OSA as evidenced by lateral cephalometric radiographs with tracings/measurements/predictions or by 3D CT airway imaging.AHI/RDI criteria as specified

Documentation must include imaging analysis and demonstration that non-surgical treatments have failed or were not tolerated; discuss weight loss if member is obese.

Other skeletal airway abnormalities: Covered when there is documentation of clinical history, photographs, Panorex and cephalometric radiographs (lateral and PA where indicated) with analysis, and any other supporting imaging or tracings that support the treatment plan.

Speech/intelligibility-related surgery

Covered when functionally indicated and non-surgical therapies have failed

Surgery for poor speech intelligibility: Surgical procedures to improve speech intelligibility are covered when the member continues to have poor intelligibility when speaking in sentences after failure of all other modalities. Required documentation includes history, examination, diagnosis, proposed treatment plan with expected improvement, and a Speech-Language Pathology (SLP) evaluation attesting to the degree of impairment, prior failed interventions, and expected benefit from surgery.failed non-surgical modalities and SLP evaluation

Tumor, fractures, osteoradionecrosis

Covered when documented evidence of pathology is provided

Hospital-based oral surgery for serious medical conditions

Coverage determination

Policy coverage is determined by meeting InterQual criteria for applicable services or, when InterQual criteria are not applicable, by meeting consensus guideline‑derived criteria.

General coverage logic: When InterQual criteria are applicable for a specific oral/maxillofacial service, those InterQual criteria determine medical necessity. For services not covered by InterQual, criteria derived from consensus guidelines (for example, ACPA, AAOMS, van der Molen et al.) will be applied to determine medical necessity.InterQual or consensus-guideline criteria

InterQual subsets should be accessed via the plan provider website; plan-specific variations (MassHealth, Medicare, OneCare, SCO) and the updated prior authorization table must be considered when adjudicating authorization requests.

Mass General Brigham Health Plan does not provide coverage for procedures that are performed solely to enhance appearance in the absence of any signs or symptoms of functional abnormality or associated medical complication. Such procedures are considered cosmetic and are not covered unless the coverage criteria explicitly state otherwise. Additionally, expenses associated with the orthodontic phase of care (both pre‑surgical and post‑surgical) are considered dental in nature and not covered under the medical benefit, except when they are consequent to treatment of cleft lip/palate in a Commercial member under age 18.

Prior authorization is required for many oral and maxillofacial procedures for adults and for certain member types. Providers must obtain prior authorization per the plan's updated prior authorization table and plan-specific variations (OneCare, SCO, MassHealth, Medicare). Failure to follow the updated prior authorization requirements may result in claim denials or retroactive noncoverage.

Prior authorization required for oral and maxillofacial procedures for members ≥18 (commercial and most products).
Cleft lip/palate repairs do not require prior authorization for members <18; prior authorization is required for cleft procedures for members ≥18.
