A prescription for ABA from the member's primary care provider or specialty physician that includes specific treatment goals.
Lucet does not require medical necessity review with prescribing PCP/specialty physician more than annually; treatment authorization requests must include an annual review by prescribing PCP/specialty physician in consultation with habilitative services provider that documents benefit to member, identifies new or continuing goals, and develops new or continuing treatment plan.

Pre-treatment assessment authorization requires submission of documentation that establishes an ASD diagnosis by a licensed, qualified clinician and a comprehensive diagnostic evaluation ruling out alternative diagnoses, plus baseline assessments and a statement of hours needed for the assessment. Assessment services must follow applicable provider manuals, ethics codes, standards of care, and state law.

ABA targets behavioral deficits and excesses associated with ASD, seeking to increase adaptive skills (communication, social, self-help) and reduce problematic behaviors (e.g., aggression, self-injury, elopement).

ABA treatment typically occurs in structured environments with individualized treatment plans developed during pre-treatment assessment. Plans include targeted treatment goals, caregiver involvement and training, coordination of care, safety and discharge planning, and periodic reassessment; a Functional Behavioral Assessment (FBA) may be included as part of assessment to describe problem behavior, antecedents, consequences, and the function of behavior.

Service Intensity Classification

Service delivery generally follows two treatment models; provider should select model needed:

ABA services must be provided by professionals licensed, certified, or otherwise authorized under the Health Occupations Article or similar state requirements (note: Maryland law only allows Licensed Behavior Analysts to practice ABA).

Under Maryland law, ABA must be provided by professionals authorized under the Health Occupations Article (Maryland law permits practice of ABA only by Licensed Behavior Analysts (LBAs)). For Maryland members, pre-treatment requests must include a comprehensive evaluation and a prescription for ABA from the member's PCP or specialty physician that specifies treatment goals.

Lucet will not deny coverage solely because of the number of habilitative services prescribed when the prescription falls within the state-specified weekly non-denial limits: ≤ 25 hours/week for children who are at least 18 months and under 6 years, and ≤ 10 hours/week for children who have reached their 6th birthday and are under 19 years. Lucet also requires an annual review by the prescribing PCP or specialty physician (in consultation with the habilitative services provider) documenting benefit to the member, new or continuing goals, and a new or continuing treatment plan, and does not require medical necessity review with the prescriber more than annually.

Providers must include a plan for caregiver training to support generalization of skills across stimuli, contexts, and individuals, and be able to show how instructional control will be transferred to caregivers. If caregiver involvement is not successful, the provider must identify and document an appropriate alternative plan to promote generalization, including post-discharge strategies.
