inv-01: Headache-related MRI indications

Covered when ANY of the following headache-related conditions are present

Headache Red Flags and New Features: Chronic headache with a change in character or pattern (e.g., more frequent, increased severity, or duration) not explained after evaluation of common causes (e.g., medication overuse syndrome or cervicogenic headache) and failure to respond to standard medical management; suspected aneurysm rupture/AVM (typically thunderclap or new onset <48 hours); prior history of stroke or intracranial bleed with sudden severe headache; onset of headache before age 6 years; new onset after age 50 years not explained after evaluation of common causes; clinical signs/symptoms strongly suggesting metastatic cancer; acute, new, or fluctuating neurologic deficits (unilateral sensory deficits, unilateral limb weakness, speech difficulties, visual loss, lack of coordination, gait disturbance, seizures, otherwise unexplained vomiting, acute hypertension, cranial nerve abnormality, mental status changes, papilledema or other signs of increased intracranial pressure); significantly immunocompromised patients; pregnancy or post-partum; risk factors for cerebral venous thrombosis (e.g., known active coagulation disorder); reproducible exertional/positional/sexual/Valsalva-associated headache; fever or meningismus with suspected CNS cause; pediatric-specific red flags such as occipital location, age <6 years, or headache that repeatedly awakens the child from sleep

Seizure indications: New onset seizure (first focal seizure or first unprovoked generalized seizure); newly identified change in seizure activity or pattern not otherwise explained; medically refractory epilepsy; seizure in a child younger than 2 years (excluding febrile seizures); preoperative evaluation when surgery is being considered

inv-04: Surveillance imaging (MS, tumor)

Covered when ALL of the following are met for surveillance

MS and tumor surveillance: Imaging performed for surveillance is consistent with neurology/oncology management: annual imaging to monitor for new lesions for multiple sclerosis after discussion with neurology; short-term follow-up allowed after clinical symptoms of a flare or radiologic evidence of flare (including 3-6 months after radiologic flare); imaging 3-6 months and/or 6-12 months after changing a disease-modifying agent; routine surveillance of previously diagnosed brain tumor based on the treatment plan from neuroscience specialty or oncology; follow-up imaging after treatment/procedure when documentation clearly indicates medical reason for additional imaging

MRI is not indicated for headache diagnoses of migraine, cluster headache, tension-type headache, or chronic stable headache when there are no focal neurological deficits on clinical examination. Requests for imaging in these scenarios should be evaluated against the policy criteria for red-flag features before approval.

Routine brain MRI for common primary headache diagnoses—specifically migraine, cluster, tension-type, and chronic stable headache—is considered not indicated when focal neurologic findings are absent. Routine imaging solely for these primary headache types without red-flag signs is therefore not supported by the medical necessity criteria.

Denial Risk

Headache without focal deficits

Brain MRI for patients with headache (eg, migraine, cluster, tension‑type, or chronic stable headaches) without new or progressive focal neurologic deficits is generally not indicated. Consider MRI only if one or more of the following are present: abnormal neurologic examination/localizing signs, new onset or change in headache pattern, progressive or late‑onset headache (>50 years), signs of raised intracranial pressure (eg, papilledema), focal seizure, or systemic signs suggesting secondary cause. If imaging is requested for headache, submit documentation supporting these findings.

Not indicated for routine evaluation of stable primary headache without focal deficits.
Indications that may justify MRI: new focal deficit, new or progressive headache pattern, red flag symptoms (eg, fever, weight loss), papilledema, focal seizure, onset >50 years.

Documentation Required

Pre-imaging clinical evaluation

Obtain and submit a prior clinical evaluation before imaging. For movement disorders, dementia/cognitive decline, seizure evaluation, or other neurologic complaints, document prior medical evaluation such as medication review, exclusion of metabolic or reversible causes (thyroid, B12, electrolytes, LFTs, CBC), and neurologic assessment. For movement disorders, document why MRI is needed (eg, atypical features, new non‑Parkinson symptoms, pre‑surgical evaluation). Include the last 6 months of clinical notes from the requesting provider and/or specialist to support medical necessity.
