Coverage Criteria — Trilogy and Home NIPPV

Initial therapy for progressive neuromuscular disease

Covered when ALL of the following are met:

Diagnosis: Diagnosed progressive neuromuscular disease (e.g., muscular dystrophy, myasthenia gravis, polio) resulting in respiratory insufficiency

Physiologic criteria (any one required): Mechanical ventilation required due to respiratory insufficiency demonstrated by ANY ONE of the followingsee child nodes

Contraindication exclusions: No alteration in level of consciousness; no anatomic abnormality that precludes mask fitting; no excessive secretions; no impaired cough or inability of mechanically assisted cough to clear secretions; no swallowing disorders; individual capable of protecting airway

All absence of listed contraindications required

Requests for Trilogy ventilators for diagnoses other than amyotrophic lateral sclerosis (ALS) or other neuromuscular disorders should be discussed with the on‑call TSPMG Pulmonologist (Chief or designee) prior to approval. Trilogy ventilators are typically ordered through Pulmonary Care Services and, when used for ALS, may be approved when recommended by TSPMG Pulmonology or the Emory ALS Clinic.

Home non‑invasive positive pressure ventilation (NIPPV) is not medically necessary if ALL of the medical appropriateness criteria are not met. The required criteria include a diagnosis of progressive neuromuscular disease causing respiratory insufficiency, demonstration of mechanical ventilation requirement by any one of the physiologic thresholds (nocturnal arterial O2 saturation < 88% for 5 consecutive minutes, arterial PCO2 ≥ 45 mm Hg, forced vital capacity < 50% predicted, or maximum inspiratory pressure < 60 cm H2O), and absence of listed contraindications (e.g., altered level of consciousness, anatomic mask‑fit issues, excessive secretions, impaired cough not cleared by assisted cough, swallowing disorders, and inability to protect the airway).

Requests for home non-invasive positive pressure ventilators (e.g., Trilogy) must include documentation that the member has a progressive neuromuscular disease and meets objective respiratory criteria for respiratory insufficiency. Requests for Trilogy ventilators for diagnoses other than ALS or other neuromuscular disorders should be discussed with TSPMG Pulmonology (on-call pulmonologist or Pulmonology Chief/designee) prior to approval.

Clinical documentation must demonstrate: 1) a diagnosed progressive neuromuscular disease (e.g., muscular dystrophy, myasthenia gravis, polio, ALS); and 2) objective evidence of respiratory insufficiency defined by ANY ONE of the following: arterial O2 saturation <88% for 5 consecutive minutes during nocturnal oximetry; arterial PCO2 ≥45 mm Hg (6.0 kPa); forced vital capacity (FVC) <50% of predicted; or maximum inspiratory pressure <60 cm H2O. Documentation should also confirm absence of contraindications (no altered consciousness, no anatomic mask‑fitting barriers, no excessive secretions or unmanageable secretions despite mechanically assisted cough, no swallowing disorders) and that the individual can protect their airway.
