Evidence reviews find the overall quality of published literature on dry needling (DN) for myofascial pain syndrome (MPS) to be low or insufficient. The Medical Technology Assessment Committee concluded there is insufficient published evidence to determine that DN is superior or equivalent to acupuncture, injections, or other therapies for reducing pain or improving range of motion; trials comparing DN to sham needling are conflicting and provide at best weak support for benefit in some settings. For neck and trapezius pain specifically, a low‑quality body of evidence indicates DN appears to be safe and may be somewhat effective when combined with exercise, but it is unclear whether DN adds benefit beyond standard therapy or exercise alone. Several trials showed improvements from DN plus exercise compared with pretreatment baseline, yet sham‑controlled and comparative studies produced mixed results and did not consistently demonstrate clinically meaningful additional benefit. Important uncertainties remain about optimal treatment parameters (number of points, duration of relief, and retreatment intervals).

For Non‑Medicare Kaiser members, dry needling is designated not medically necessary because the published evidence is insufficient to demonstrate that the procedure is as safe and effective as established alternatives. Claims for DN in Non‑Medicare plans are subject to denial on that basis.

Providers should verify benefit and authorization rules for specific plan types and codes, but where Non‑Medicare coverage applies, DN is considered not medically necessary and is not covered under this policy.

Dry needling should be used as part of a broader treatment plan. Post-procedure exercises and stretching are recommended to maximize and maintain benefit.

Recommend stretching, exercise, ergonomic adjustments, or other rehabilitation after needling to restore range of motion and reduce recurrence of trigger points.
Document prior and concurrent conservative therapies (exercise, physical therapy, heat, topical NSAIDs, massage, spray-and-stretch) and plan for adjunctive therapy when requesting coverage.

Myofascial pain syndrome (MPS) is a common musculoskeletal pain condition originating in muscle or related fascia, characterized by myofascial trigger points (MTrPs)

Dry needling (DN) is a procedure in which an acupuncture‑style needle is inserted into an MTrP without injection of fluid to elicit a local twitch response and inactivate the trigger point. DN is distinct from acupuncture, and optimal outcomes are most often reported when needle‑elicited twitch responses are achieved and followed by stretching or exercise. Common adverse events include post‑needling soreness, local hemorrhage, and, rarely, syncope.
