Coverage Criteria for Facet Joint Procedures

Facet Injections / MBB medical necessity

Covered when ALL of the following are met

Documentation: Submit last 6 months of clinical notes from the requesting provider and/or specialist (including physical therapy notes) to support medical necessity.

See documentation requirements in the policy.

Clinical indication: Strong suspicion for facet joint–mediated pain (e.g., focal tenderness over the facet joint, pain provoked by extension/rotation, absence of radicular symptoms) or low back pain with normal imaging where facet etiology is suspected.

Clinical features consistent with facet etiology per background guidance.

Conservative therapy: Conservative treatments have been trialed for at least 3 months prior to consideration of facet injections.>= 3 months

Conservative therapy requirement described in background and conservative treatment modules.

Imaging guidance and coding: Procedures are performed with image guidance consistent with listed CPT codes (fluoroscopy or CT) and applicable fluoroscopic guidance code when required.

Facet Neurotomy / RFA medical necessity

Covered when ALL of the following are met per adopted CMS criteria

Diagnostic confirmation: Positive diagnostic medial branch blocks or intraarticular diagnostic injections consistent with facet-mediated pain per the criteria in the adopted LCD/LCA.

Diagnostic MBBs or IA injections used to evaluate candidacy prior to neurotomy.

Trial of conservative therapy: Failure of conservative care, including at least a 3-month trial, prior to radiofrequency neurotomy.>= 3 months

Conservative therapy requirement applies to neurotomy candidacy.

Evidence/adoption of criteria: Patient selection and procedural indications conform to the CMS/Noridian coverage criteria adopted by Kaiser Permanente for facet neurotomy.

KPWA adopted CMS coverage criteria for facet neurotomy; evidence base has limitations discussed in the policy.

The evidence base for radiofrequency neurotomy (RFA) for chronic neck and back pain is limited and inconsistent. Systematic and randomized trial data include a small number of randomized controlled trials with variable methodological quality; multiple reviews and committee assessments concluded that available RCTs were small, underpowered, or had design limitations. As summarized in the internal evidence reviews, the use of RFA "does not meet the Kaiser Permanente Medical Technology Assessment Criteria" for reliably improving health outcomes in chronic neck and back pain, and subsequent small trials failed to demonstrate clear benefit versus sham treatment. Given these limitations, patient selection and application of adopted CMS criteria are important when considering RFA.

Because of the uncertain long-term benefit demonstrated in the literature, this policy aligns selection and coverage with the adopted Noridian/CMS guidance rather than broad clinical permissiveness — i.e., RFA is covered only when the specific diagnostic confirmation, prior conservative therapy, and other criteria adopted from CMS are met.

Certain ultrasound-guided paravertebral facet injection codes (notably 0215T–0218T) are explicitly identified in the source document as experimental, investigational, or unproven. Conservative noninvasive treatment should be attempted before considering facet joint injections. A trial of conservative therapies (such as physical therapy, activity modification, analgesics, and other noninvasive measures) for at least 3 months is recommended prior to injection therapy.
